DATA PROTECTION AND CONSENT POLICY

The Privacy Policy of Clínica d’Avenida applies to the following companies:

Clínica Dentária da Avenida de Gaia, Sociedade Lda, with head office at Avenida Da República, no. 885 1st floor; 4430-201Vila Nova de Gaia, with the single registration and taxpayer number 502072148 (hereinafter referred to as Clínica D’Avenida)

and

Clínica Dentária da Avenida de Matosinhos, Sociedade Lda, with head office at Rua Alfredo Cunha no. 37, 2nd floor, Room 26, 4450-023 Matosinhos, Portugal, with the single registration and taxpayer number 503205389 (hereinafter referred to as Clínica D’Avenida)

collect and process personal data of its Clients, such as:

1. a) name;
2. b) date of birth;
3. c) address;
4. d) taxpayer number;
5. e) civil identification number;
6. f) telephone and mobile phone number;
7. g) e-mail;
8. h) health subsystem.

The client freely and expressly accepts and consents, by means of an unequivocal positive act, to allow the personal data requested or optionally provided by him/her, to be collected and processed by Clinica D’Avenida as provided herein.

The communication of personal data does not constitute a legal obligation, being, however, necessary so that Clinica D’Avenida can proceed to the execution of contracts for services and

respective invoicing, as well as the dissemination and advertising of marketing activities related to the activity of the Company.

Clinica D’Avenida is responsible for processing the personal data of its clients which consists of the operation or set of operations performed on those personal data, by automated or non-automated means.

In accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016, all Clinica D’Avenida Clients have the right to:

1. a) at any time withdraw consent for the processing of personal data;
2. b) proceed or request the rectification of any personal data that the Data Subject has provided;
3. c) access personal data at any time;
4. d) refuse to allow their use for marketing and/or advertising purposes;
5. e) request the portability of personal data to another entity that the client designates for this purpose;
6. f) request the erasure of all personal data provided, which is not necessary for the fulfilment of any legal obligations by the entity responsible for the respective collection;
7. g) be informed by Clinica D’Avenida if, eventually, it intends to proceed with the processing of personal data for purposes other than those for which, hereby, consent is given.
